To be completed where a student will not be attending school for a period of time due to travel.

Please complete and email or hand in at the front office.

To be completed when illness or misadventure prevents the on-time completion of an assessment.

Please complete and present with supporting documentation to the relevant Head Teacher prior to 8:50am on the first day of the student's return to school. Forms may also be emailed to the school.

To be completed where a parent/carer wishes to change the student choice for special religious education (SRE) or special education in ethics (SEE) classes. More information about SRE and SEE is on our Learning page.

To be completed where a student requires a day loan for more than two days in a week. For example, where a student's personal device is undergoing repairs.
